N-648 Medical Disability Evaluation
We provide N-648 Medical Disability Evaluations for patients applying for U.S. naturalization who are requesting an exception to the English and/or civics testing requirement due to a qualifying medical condition.
What is Form N-648?
Form N-648 is a Medical Certification for Disability Exceptions, completed by a licensed medical provider and submitted with a patient’s naturalization application (Form N-400). This form is used when a physical or mental impairment prevents an applicant from meeting the standard English and civics requirements.
Our Evaluation Process
Clinical evaluation
Conduct a clinical evaluation related to the qualifying condition.
USCIS criteria
Determine whether the patient meets USCIS criteria for a disability exception.
Form completion
Complete Form N-648 accurately and in compliance with USCIS guidelines.
If appropriate, the completed N-648 form is typically ready within one week of the visit.

- Not all medical conditions qualify for an N-648 exemption.
- Approval of the exemption is determined by USCIS, not the clinic.
- Completion of Form N-648 does not guarantee approval.

Payment Policy
Payment is due at the time of service. Evaluation fees are separate from immigration medical exams and are not covered by insurance. We accept cash and major credit cards.
